USPSTF needs overhaul, groups say

The Agency for Healthcare Research and Quality desperately needs to restructure the U.S. Preventive Services Task Force (USPSTF), which more than a dozen organizations claim suffers from a lack of transparency and nonrepresentation of key medical specialties. (Source: Health Imaging News)

Results of national audit of falls and bone health in older people

This report contains the findings from the National Audit of Falls and Bone Health in Older People, which examined the organisation and commissioning of services (both in acute and primary care) provided to older people for falls prevention and bone health, and the clinical care provided for those that have fallen and suffered a fracture. This audit was commissioned by the Healthcare Quality Improvement Partnership (HQIP) as part of the National Clinical Audit and Patient Outcomes Programme (NCAPOP).   Although some improvements have been made since previous audits, major variations between organisations persist and deficiencies in care remain widespread.  The document summarises key recommendations made in relation to the following Department of Health objectives:   ...

Positive contrast magnetic resonance bursography for assessment of the navicular bursa and surrounding soft tissues

Magnetic resonance (MR) imaging is often performed to determine the cause of palmar heel pain. We evaluated how distension of the navicular bursa affected the MR appearance of the navicular bursa and associated structures. An MR evaluation was performed on normal cadaver limbs and cadaver limbs from horses with lameness localized to the foot. The normal navicular bursae were injected with 2, 4, or 6 ml of solution. The bursae of the feet from lame horses were injected with 4 or 6 ml, and the MR study was repeated. All bursae were examined grossly to verify the presence or absence of adhesions. Clinical patients that had initial MRI abnormalities suggesting adhesions were also evaluated. Distension of the proximal recess of the normal navicular bursa, proximal to the collateral sesamoid...

Retrospective investigation of prognostic indicators for adult horses with infection of a synovial structure

Conclusions Of the variables available at presentation, only evidence of bone or tendon involvement negatively affected survival and athletic function. During treatment of synovial sepsis, analysis of synovial fluid at 4–6 days and bacterial culture results have prognostic value. (Source: Australian Veterinary Journal)

Salvage of foot with extensive giant cell tumour with transfer of vascularised fibular bone graft

We report a case with extensive involvement of all the tarsal bones and metatarsal bases in a young adult. After excision his foot was reconstructed with vascularised bone flap. We were able to save his foot after a wide local excision and reconstruction with free fibula graft. Graft united early and showed excellent remodelling because of good vascularity. We feel that this method deserves consideration as a last attempt to salvage functional foot in disease like this. (Source: Indian Journal of Plastic Surgery)

The Effects of Cortical Bone Thickness and Trabecular Bone Strength on Noninvasive Measures of the Implant Primary Stability Using Synthetic Bone Models

What constitutes “normal” 25(OH)D?

Over the past decade, we have learned that calciferol has a number of very important biologic functions beyond its skeletal effects. This was nicely reviewed in a recent Medical Progress report by Russell Chesney (J Pediatr 2011;156:698-703). This understanding has resulted in a number of studies examining the frequency of calciferol deficiency in various groups. A problem of these studies, however, has been defining “sufficiency” of this analyte. Studies that have used measures of bone turnover or parathyroid hormone as evidence of sufficiency may not reflect the levels that are “normal” for calciferol's non-skeletal effects. Vitamin D2 or vitamin D3 supplementation in daily dose: Are they equivalent?

The objectives of this study were 1: to compare the efficacy of daily doses of 800UI D2 and D3 in healthy women >65years. 2: to evaluate the effectiveness of the administration of vitamin D, both in drops and tablets. Population and methods: 27 ambulatory women, mean age (X±SD) 77.1±6.8years in winter. Serum calcium, phosphorus, 25OHD, bone alkaline phosphatase and urine calcium/creatinine were evaluated at baseline and 7, 28 and 45days after supplementation with two daily regimens: G1: 800IU D3 (tablets) G2: 800IU D2 (drops). Statistics: SPSS 11.0 Chicago. IL, USA. Results: at baseline 25OHD levels (ng/ml) were similar in both groups (G1: 14.0±4.8 G2: 12.6±4.5; ns). 91.7% of the population showed deficient 25OHD levels ( (Source: Bone)

Short implants in posterior jaws. A prospective 1-year study.

Conclusions: One year after loading, 7 mm short implants provided good success rates (95% at patient level and implant level) suggesting that the use of short implants is a viable concept, however longer follow-ups are needed to confirm these preliminary results. Spontaneous bilateral distal ulna fracture: an unusual complication in a rheumatoid patient

We report a case of bilateral stress fractures of the ulna in an elderly patient with rheumatoid arthritis, and literature on this condition is reviewed. Prompt recognition and activity modification are essential to treat this rare injury. Recovery can take up to 12 weeks. Vitamin D2 and D3 supplementation in healthy adolescents from Ushuaia, Argentina

In order to improve vitamin D status of healthy adolescents from Ushuaia (55° S), three supplementations of 100,000IU of vitamin D2 (ergocalciferol) or vitamin D3 (cholecalciferol) were administered every three months: during fall (March), winter (June) and spring (September). Serum 25-hydroxyvitamin D (25OHD) was measured before the study, a month after the first supplementation, and two months after receiving the third supplementation (September 2008, April 2009 and November 2009). We studied 34 healthy adolescents, age (mean±SD) 18.5±2years old, 23 girls and 11 boys. None had received previous vitamin D. Seventeen adolescents received vitamin D2 and 17 received vitamin D3. Basal levels of serum 25OHD were 13.4±6.3ng/ml (range 4–26) in September/spring 2008 (D2 group: 14.6±6.4, D3...
